Hi Semina,
Send an urgent email to CIC and let them know that u never recd the medical forms. Send them an email too if u like. That's what I had done. They will then email u back and ask u to send them 2 photos per family member. Thats it. U should receive fresh medical forms within 2-3 weeks.
